I understand I am providing temporary care for this animal for Friends of Animal Control-Campbell County. I understand that neither FOAC nor Campbell County can guarantee health or temperament and agree to hold FOAC and/or Campbell County harmless for any damages caused to my home or property or to property, pets, or people while the animal is in my care. I further agree that neither myself nor my family nor any descendants of my family will pursue action against FOAC or Campbell County in the event there is damage caused by this animal. There is no clause in my housing agreement stipulating that pets are not allowed. All members of my household agree to foster this pet. Veterinary care for this pet will be provided by FOAC as long as I contact them prior to treatment. If I choose to pursue treatment at a veterinary clinic without first asking permission, it is possible I may not be reimbursed for my expenses. Routine preventive care (i.e., flea/tick prevention, heartworm prevention, parasite treatment) will be provided by FOAC-Campbell County. Should this foster pet be diagnosed with catastrophic medical problems, potentially fatal disease, or be injured to the extent that major medical costs will be involved in saving its life, the decision of whether to treat or not lies solely with representatives of Friends of Animal Control-Campbell County.

In accordance with Virginia Code § 3.2-6503. “Care of companion animals,” I agree to provide for my foster animal:

1. Adequate feed;

2. Adequate water;

3. Adequate shelter that is properly cleaned;

4. Adequate space in the primary enclosure for the particular type of animal depending upon its age, size, species, and weight;

5. Adequate exercise;

6. Adequate care, treatment, and transportation; and

7. Veterinary care when needed to prevent suffering or disease transmission, by first contacting a representative of Friends of Animal Control-Campbell County.

In addition, I agree:

8. This pet will be fed only high-quality name-brand food. Beneful will never be fed, nor grocery store brands of dry kibble.

9. There is an adoption fee for anyone who wishes to adopt this pet.

10. Applications will be reviewed and my opinion counts when my foster pet is going to be adopted.

11. I will not place this foster dog or cat in a home without first asking for an application to be submitted. Before placement, a contract will need to be signed by the person who is adopting. I understand the importance of the hold harmless portion of this contract.

12. I will not advertise this pet for sale on any yard sale, craigslist, or facebook page.

13. This dog will be leashwalked only, and not allowed to run free at any time. Supervision will be provided at ALL times.

14. This dog will not be left in the care of others without permission from the foster coordinator or a FOAC board member.

15. I agree to use recommended leashes/collars/harnesses for exercising my pet. No flexi-leashes will be used.

16. I agree to transport to adoption events.

17. Medicine will be given to the pet as prescribed by the FOAC members or attending veterinarian.

I agree to abide by these guidelines. I have never been convicted of animal cruelty, neglect, or abandonment, and no one in the foster household has been convicted of animal cruelty, neglect, or abandonment. If at any time this changes, I agree to notify Friends of Animal Control-Campbell County.
